Log in to the Admin of your site. If you need help finding the right place to do so, follow the instructions in the Log In article.
Go to Theme Editor>Forms in the left menu.
Add a New Form
After accessing the Form Manager, you can easily add new forms to your site. The Form Manager shows your list of forms as well as the Title, Shortcode, Date and Action Icons for each row.
Click the Add New button at the top of the window.
Type a title for the new form in the text box where it says Enter Form Title Here. This title will be how the form will be listed in the Form Manager.
Next, you will need to choose how you want to create a new form. There are two options:
Use Form Builder—This option allows you to create your form using drag and drop fields. This is a more intuitive option relying on visual fields to build the form.
Use Advance Form Editor—This option is more text field oriented and you will not be able to see a visual representation of the form you are building.
Click the green Create button. If you are successful, the Form Editor will open for the form you just created. If you click outside of the Add New Form box, or click the X in the corner of the box before clicking the Create button, it will close without creating the form.

Delete a Form
In the Form Manager, you can delete forms that have been created on your site.
Find the form that you want to delete in the list.
Select the form in the checkbox to the left of the form Title, and then click the Delete icon on the right side of the row under Action. Or simply click the Delete icon on the row of the form you wish to delete.
Note: If you select a form and then click the Delete icon in the row of a different form, the form in the row of the Delete icon you selected will be deleted.
A Confirmation box will open. If you are sure you want to delete the form, click the blue Confirm button. If you do not want to delete the chosen form, click Cancel.
Note: At this time, deleted forms may not be recovered, viewed or accessed after you have clicked Confirm to initiate the delete process.
Search for a Form
If you have a large number of forms on your site, it might be more efficient to find a form by searching for key words in the form Title.
Type a part or all of the Title of the form in the Search box.
Press the Enter key or click on the Search icon. Only forms that meet your search criteria will show up in the form listing.
Add a Form to your Website
Add a form to your website by copying the Shortcode and pasting it into a post, page or text widget. The shortcode for each form is found in one of two places:
The shortcode can be found in the Form Manager which is accessed directly under Theme Editor/Forms.
The shortcode can also be found in the Form Editor under the form title, as shown in the image below. The Form Editor is accessed from the Form Manager by clicking on the Edit Icon of the desired form.
