Electronic 1095s

The IRS requires electronic filing of 1095s in certain circumstances. You can create file in FCA which can be uploaded to EFileMyForms or Quarterly Express to be transmitted to the IRS.

To create a 1095 file:

1. Select Reports>Electronic Tax Reports>1095.
2. Select the desired provider and year and click OK.
3. Right click and Select All to select all employees and click OK.
   
4. Click Browse to browse to the desired folder.
   
5. Enter the desired File Name and click Save.
   
6. Verify the path and filename and click Next.
   
7. Verify the information on the Electronic File 1095 Export dialog.  Check the Employer Self Insured box, if needed. Select the appropriate Plan Start Month and click Next.
   
8. Enter any missing Address, City, State, or Zip information and click Finish.
   
9. The following dialog will appear when the file has been created.  Click OK to close this window.
   
10. Login to the appropriate transmitter's website and follow the directions to upload and transmit the file.
